When most people think of Botox, they picture a quick forehead treatment or a few injections between the brows. What they probably don’t picture is a syringe aimed considerably further south. Yet that is precisely what a growing number of men are reportedly doing, and some of them are talking about it quite openly, according to Allure.

Australian influencer and former ‘Dancing with the Stars’ contestant Harry Jowsey brought the topic into the mainstream when he revealed the procedure on his podcast ‘Boyfriend Material.’ Jowsey shared that he had received Botox injections in his most sensitive area, saying a friend of his had done the same. “I actually got Botox injected into my penis,” he told listeners, explaining that the treatment is said to relax the muscle. “It’s very smooth now, which is good,” he added. He also mentioned that the injections can be applied to the scrotum, though he chose to skip that particular step.

Unsurprisingly for those who follow his extensive biohacking experiments, tech entrepreneur Bryan Johnson has also undergone the procedure. Johnson, who is known for his obsessive pursuit of physical optimization, shared his experience on social media platform X in 2024. “It increases penis length by 1 cm,” he wrote, adding that the injections can also improve “erection hardness, peak systolic velocity, end diastolic velocity, and sexual health satisfaction.” Johnson later combined the Botox with shockwave therapy, claiming the combination had reduced the biological age of his nighttime erections to that of a 20-year-old.

So what does the medical community actually make of all this? Dr. Roy Kim, a board-certified plastic surgeon based in San Francisco, confirms that Botox injections can produce what he describes as a “modest improvement” in size. “If Botox is injected into the smooth muscle area of the blood vessels, the penis can appear larger, but only by about one centimeter,” he explains. As for Johnson’s claims about biological age, Dr. Kim notes that a healthy lifestyle involving regular exercise and avoiding smoking and alcohol can improve penile health, suggesting that Johnson’s broader wellness regimen likely plays a significant role in his results. “I’m not sure how many men try to optimize their health first before considering Botox, low-dose Cialis… but yes, those treatments help,” Dr. Kim said.

Genital injections of various kinds are more common than most people realize. Surgeons have been injecting hyaluronic acid and biostimulatory fillers into penile tissue for some time to add volume, typically in an off-label capacity. With Botox specifically, the motivation is “mostly cosmetic,” according to Dr. Andrew T. Cohen, a board-certified plastic surgeon from Los Angeles. “Botox can relax certain superficial muscles in the genital area, which can give the skin a smoother appearance,” he explains.

Beyond aesthetics, there are potential medical applications worth noting. Dr. Kim points to research suggesting the injections may help with erectile dysfunction by relaxing the tissue that fills with blood during an erection, thereby improving circulation and allowing for stronger or longer-lasting results. He also references a 2024 study showing encouraging outcomes for premature ejaculation, as Botox injected near the perineal muscle can reduce the force of muscular contractions during ejaculation and delay the process.

When performed correctly, Dr. Kim estimates the effects last somewhere between three and six months. Dr. Sean Doherty, a board-certified plastic surgeon from Boston, recommends that patients considering the procedure for any medically related concern consult a urologist rather than going straight to a cosmetic provider. Potential risks include bruising, swelling, discomfort, a temporary reduction in erectile function, and infection. Dr. Cohen is equally clear that this is not something to be done casually. “The anatomy in this region is complex, and treatments should always be performed in a medical setting with careful patient evaluation beforehand,” he says.

The scrotal version of the treatment, informally dubbed “Scrotox,” is gaining attention as well. According to Dr. Doherty, it works by relaxing the dartos muscle in the scrotum to reduce wrinkling and create a smoother appearance. He also notes that athletes sometimes use it to reduce sweating and friction in high-temperature conditions.

“Penistox,” as Dr. Kim calls it, remains a niche trend for now. He draws a comparison to grooming habits that were once considered unusual and eventually became mainstream, noting that he does not currently offer the procedure in his practice and has yet to have a single patient ask for it. Dr. Cohen agrees, describing both treatments as “relatively niche procedures” that will likely remain in the hands of experienced specialists. Still, as more men speak openly about the treatments they pursue, others may feel more comfortable exploring similar options. “Male aesthetics is one of the fastest-growing segments in aesthetic medicine,” Dr. Cohen observes. “As the stigma around aesthetic treatments for men fades, it’s not surprising that interest is expanding into areas like intimate wellness.”

Botox was originally approved by the FDA in 1989 strictly for eye muscle disorders, and it took over a decade before it received cosmetic approval for frown lines in 2002. The global market for neurotoxin injectables is now worth several billion dollars annually, which makes the idea of entirely new treatment areas emerging feel less like a novelty and more like an inevitable expansion. The smooth muscle tissue targeted in these penile procedures is the same type found in blood vessel walls throughout the body, meaning the underlying science of how Botox works there is not entirely different from how it functions in more familiar cosmetic applications.
